Positive Coaching

In AYSO, Coaches and Referees are teammates who work together to ensure a safe, fair, and fun experience for all players and parents.

Coach Training requirements

Every AYSO Coach must be registered as a Volunteer for the current year, complete the AYSO Safe Haven on-line course, the CDC Concussion on-line course, the Sudden Cardiac Arrest course and the SafeSport course. To comply with California law, coaches take a background check and get fingerprinted.

Additionally:

  • U6 Coaches must complete the U6 Coach Course (2 hours) (instructor led preferred, online as a backup option).
  • U8 Coaches must complete the U8 Coach Course (2 hours) (instructor led preferred, online as a backup option).
  • U10 Coaches must complete the U10 Coach Course (3 hours) (instructor led preferred, online as a backup, online also requires instructor lead U10 Field Training).
  • U12 Coaches must complete the U12 Coach Course (6 hrs).
  • U14 Coaches must complete U12 (6 hours) and the Intermediate Coach Course (15 hours). It is best to coach at least one season between U12 and Intermediate. 
  • U16 & U19 Coaches must complete U12 (6 hours), Intermediate (15 hours), and the Advanced Coach Course (18 hours). It is best to coach at least one season between Intermediate and Advanced.
In order to coach their team, every Coach must be authorized by Millbrae AYSO RCA.

Incident

INCIDENT REPORT FORM

The AYSO Incident Report Form is used whenever there is a personal injury, damaged property, or threats of or actual physical violence surrounding an AYSO game, practice, event or property.  The form should be prepared by the Coach, AYSO Official, or AYSO Volunteer which may be a member of the regional staff such as the regional safety director, or by tournament or event staff members. Note:  This form should NOT be completed by a parent unless the parent is the Coach.

Release Form to return to play

AYSO Participation (Return to Play) Release Form

The AYSO Participation (Return to Play) Release Form not only protects AYSO, but more importantly, the participants. Players should not participate until they are fit to play again. The form requires a parent or guardian signature in order to help ensure a player is well enough to return to participation. A Participation (Return to Play) Release Form must be signed by parents/guardians whenever there is an injury or illness requiring medical treatment including serious illnesses like hepatitis and pneumonia. A signed Participation (Return to Play) Release Form is required whenever a player is removed from play for a suspected concussion. A medical release signed by the appropriate medical professional must also be attached to the Participation (Return to Play) Release Form before a player can return to play after a concussion is suspected.
